Option 2: Paper Form

  • Fee: $3.00 per document.
  • Payment: Cash, Cashier's Check, or Money Order (payable to "Office of the Lieutenant Governor"). No personal checks.
  • Mailing Address:
    Office of the Lieutenant Governor

    415 S. Beretania Street
    Honolulu, Hawaiʻi 96813

Important Notes

  • Birth/Marriage/Death Certificates: Must be issued by the Hawaii Department of Health. If issued before 2025, you may need a new copy.
