Step 2 — Fill in Details

Enter the Requestor Information (you) and the Document Usage Information (destination country).

Step 3 — Pay & Print

Pay the $25/doc fee online via credit card or e-check. Print the "Service Request Confirmation" page. This is your form.

What to put in the envelope:

  1. Service Request Confirmation Page — The one you printed after paying.
  2. Your Document(s) — Must be originals or certified copies.
  3. Return Envelope — Self-addressed with postage.

Common Rejection Reasons (NJ-Specific)

  • Local Vital Records — Sending a document from a specific town hall instead of Trenton.
  • Remote Notary — Documents notarized via webcam/remote online notary (RON) are REJECTED.
  • No Confirmation Page — Sending documents with just a check and no online receipt.

Frequently Asked Questions

Can I just send a check?

You shouldn't. The online system is the preferred and fastest method. Sending a manual check often leads to delays if the form isn't perfect.

How long does it take?

Mail processing takes about 12-20 business days. Walk-in service is same-day or next-day but has limited hours (8:30-2:00).
